Open Door Family Medical

Open Door Family Medical Centers has been serving low-income, under-served residents and those lacking access to health care in the communities of Westchester and Putnam counties for more than 40 years, providing high quality primary health care and human services at affordable prices.  

This year, Open Door will be the medical home for more than 50,000 patients, providing more than 250,000 visits. Our family medical services include primary medical care, dentistry, mental health, optometry, podiatry, and women’s health in Ossining, Sleepy Hollow, Port Chester, Mount Kisco, and Brewster, NY.  

Open Door Family Medical Centers is seeking a Licensed Practical Nurses (LPN) to join the busy and dedicated Port Chester, NY location. The Licensed Practical Nurse will promote and restore patient’s health by completing the nursing process, collaborating with the Nurse Practitioners and Patient Care Technicians to provide comprehensive quality care and support to patients and families. He/she will participate in cross-functional care teams.  

All Open Door Family Medical Centers are located in Westchester and Putnam counties located just North of New York City, which offer excellent schools, easy access to a multitude of outdoor activities, and superb transportation connections to New York City.  

Responsibilities:  

• Performs and accurately records screening tests (including but not limited to blood pressure, EKG, Spiro, vision, hearing) to patients of neonatal, pediatric, adolescent, adult and geriatric age groups.
• Encourages preventative care to patients of all ages.
• Motivates patients and families by giving support, showing interest and conveying understanding.
• Participates in patient counseling and education activities related to primary health care, HIV/STD prevention and treatment, child/adult health maintenance, and family planning.Documents necessary information in patient’s electronic medical record.
• Prepare and maintain documentation in compliance with JCAHO standards and applicable city, state and federal laws.
• Administers medications and treatments as authorized by the provider.
• Performs phlebotomy and simple laboratory procedures including but not limited to rapid strep test, urinalysis, pregnancy and tuberculosis screening.
• Assist health team in follow-up of missed appointments and non-complaint patients by telephone and mail.
• Maintains patient confidentiality at all times as per Open Door policies and procedures.
• Review patient record one-day prior to scheduled visit to assure chart preparedness.
• Maintaining current up to date immunization record through NYSIIS.
• Follows current ACIP guidelines.
• Participates in the storage and maintenance of medications and immunizations.  

Requirements:   

  • Graduation from an approved school of practical nursing  
  • Associates Degree in Nursing preferred  
  • Current Practical Nurse License in NYS  
  • 2-5 years previous experience in nursing preferred  
  • Bilingual (Spanish) Preferred  
  • Current CPR Certification  
  • Phlebotomy/blood draw experience.  
  • Knowledgeable of the nursing process  
  • Outstanding written and verbal communications skills  
  • Ability to work in a culturally diverse patient and staff environment  
  • Ability to work in a collaborative team-based care environment  
  • Highly organized, flexible, and self-directed, able to multi-task  
  • Responsive and comfortable working in a fast paced environment  
  • Excellent judgment skills and ability to prioritize tasks
